Blank Forms For anyone who does not receive a Census form in the mail, blank forms are available at the Livingston County Chamber of Commerce from 10 a.m. to 1 p.m. every day until April 19.  A Census employee is at the Chamber building at that time to assist with Census questions and to provide blank forms when needed.
