Town Clerk Accepting Applications for Heroes Wall

Thank you for your interest in participating in our Hometown Heroes recognition wall. The wall will recognize currently serving military personnel who are from the Town of Livonia. The Town of Livonia includes: the Village of Livonia and the hamlets of Lakeville, South Lima, South Livonia, Livonia Center and Hemlock.

Please complete the application form and submit it along with a 5x7 photo of your nominee in uniform to: Colleen West, Livonia Town Clerk, PO Box 43, Livonia, NY 14487.

The Town of Livonia will compile the photo, biographical information and icon of the nominee's branch of service in an 11 x 17 frame to display on our wall. If you wish for your original photo to be returned to you, please indicate that on the nomination form and provide the correct name and address to where the photo should be returned.

All data furnished by the applicant(s) may be edited due to space configurations. The Town of Livonia assumes no responsibility for unverified data, as furnished.
